General provisions

1. These Regulations can be found on the following website http://www.fizjoandwell.pl in a display case in front of the entrance to the surgeries and can be made available for inspection at the patient's express request.
2. Physjo&Well - the physiotherapy and massage surgeries are open Monday to Friday from 8:00 a.m. to 9:00 p.m., on Saturdays from 8:00 a.m. to 5:00 p.m. and on Sundays, holidays and public holidays by appointment.
3. The place where physiotherapy services are provided is the KEN Center Gallery at 15 Jana Ciszewskiego Street, 3rd floor, surgeries 8 and 9 or the place where the patient calls (home visits).
4. Both the Fizjo&Well surgeries and the equipment used in it meet the requirements to be met by physiotherapy professional practice providers.

Booking and cancellation policy

1. Appointments can be made 24 hours a day via the website, at: www.fizjoandwell.pl , through the booksy application and by phone: +48 669 587 257 from 8:00 a.m. to 8:00 p.m., Monday to Friday. Reservations can also be made in person at the office: Galeria KEN Center at 15 Jana Ciszewskiego Street, 3rd floor, offices 8 and 9 in Warsaw.
2. If you are unable to attend your appointment more than 24 hours before the appointment, you can cancel or reschedule at any time by phone or text message on +48 669 587 257 or by cancelling/changing your appointment on the booksy app.
3. If an appointment is cancelled less than 24 hours before the appointment, the Patient is obliged to pay the a handling fee of 100% of the visit price. The payment will be added to the bill during the next visit or is payable by bank transfer no later than 7 days from the date of the scheduled visit. In the case of failure to cancel an appointment and failing to show up for it, the Patient is obliged to pay the 100% visit prices. The payment will be added to the bill during the next treatment or is payable by bank transfer no later than 7 days after the scheduled treatment.
4. By making an appointment, you are deemed to have read and accepted the provisions of these regulations.

Payment

1. Rates for visits are billed according to the price list available at http://www.fizjoandwell.pl/cennik.
2. Rates for services provided on Sundays, holidays and public holidays are charged twice.
3. Payment for services carried out in-house at the Fizjo&Well practice can be made in cash, by electronic transfer or by BLIK quick transfer to your phone.
4. Payment for services provided at the point of call (with travel to the customer) is possible by bank transfer or BLIK transfer to the telephone. It is not possible to pay in cash for services with travel to the customer.

Gift voucher

1. A gift voucher is a named gift card that can be used by the recipient, indicated by name on the voucher.
2. The gift voucher can be purchased in person at Fizjo&Well at 15 Ciszewskiego Street, floor 3, office 8, by phone at (+48) 669 587 257 and by email: kontakt@fizjoandwell.pl.
3. The gift voucher is valid for 12 months from the date of purchase.
4. The gift voucher is redeemed at any time by prior appointment in the manner of your choice - by phone or online at www.fizjoandwell.pl
5. The voucher purchased is non-refundable. It is not possible to redeem the voucher beyond the expiry date.
6. A gift voucher can be purchased for a single treatment, for a series of treatments or for a specific amount to be used by the recipient.
7. It is possible to change to any treatment of the recipient's choice of the same value or to a more expensive treatment upon payment of the remaining amount.
8. It is possible to send the voucher by post to the address indicated for an additional fee.
